I will provide a free, initial 30 minute consultation to help identify and find the services you need at this most important time.

Moving forward, if you feel we are a good fit,  I will provide a detailed report of our discussion and referrals if requested and next steps to take. Planning for death when you are healthy means there is less to think about if you get sick. Coming to terms with your own mortality can help you live life to the fullest now.

• Creating a Advance Directives that reflects subjective wishes for what you want, and don't want at the end of life
• Choosing the right Health Care Power of Attorney who will ensure your wishes will be honored
• Facilitating a family meeting to help your family understand your wishes
• Creating a plan for how you want to spend your final days and communicate them to your family
• Education on services you may need at end of life

There are 5 categories that need to be addressed to have a complete plan for end of life and should be done ahead of time. They address all physical, mental, emotional, spiritual, and financial needs for care. You can't make good decisions when you are in crisis or pain. This package provides education, discussion, and planning for the following end-of-life aspects:

This process may take up to 10 visits depending on your needs.
