Basic Peace of Mind Planning

Services include:

  • Creating an Advance Directive reflecting your end-of-life wishes
  • Choosing a Health Care Power of Attorney to ensure your wishes are honored
  • Facilitating a family meeting to communicate your wishes
  • Creating a plan for your final days and sharing it with your family
  • Education on end-of-life services you may need

Fees can be customized to fit your needs. In-person or tele-doula services available.

Complete Peace of Mind Advance Planning

5 categories need to be addressed to have a complete plan for the end of life, which should be done ahead of time. They address all physical, mental, emotional, spiritual, and financial needs for care. You can’t make good decisions when you are in crisis or in pain. This package provides education, discussion, and planning for the following end-of-life aspects:

  • Living Will
  • Health Care Power of Attorney
  • Physicians Orders for Life-Sustaining Treatment
  • Will and Trusts
  • Organizing personal documents
  • After Death Plan
  • This process may take up to 10 visits depending on your needs.
